For most new patients (excluding pediatrics), your first step is a Patient Navigation appointment—a personalized visit designed to help you feel confident and prepared from day one. During this one-on-one meeting, you’ll get to know our services, learn how to access care, and connect with a team member dedicated to setting you up for success.
After your Patient Navigation visit, you’ll also have access to our same-day clinic for acute needs when your provider isn’t available. Every step of the way, we’re here to make getting started easy and supportive.

Preparing for your first visit.
Planning ahead can help you get the most out of your appointment. Please bring:
- A picture ID and insurance card(s)
- Medical or dental records, including immunization history, past medical history, family history, social history, recent lab results, procedures, ER or urgent care visits, and dates of recent hospitalizations
- Guardian or custody papers if you are a foster parent, grandparent, or adult guardian (such as a Power of Attorney)
- Your current medications or a complete list of them
- Arrive 15 minutes early to allow time for check-in
- Make a list of any concerns or questions you’d like to discuss — this helps us make sure we address everything that matters to you
- Don’t hesitate to ask questions during your visit — your care team is here to listen, guide, and support you every step of the way
