Case Management
At Kaniksu Community Health, Case Management is a voluntary service designed to complement the behavioral health care you already receive. It provides ongoing support through regular check-ins to help you track your progress and achieve your goals in areas like basic life skills, employment, support services, and access to vital community resources.
We understand that finding and coordinating the right community resources can sometimes feel overwhelming. Our professional Case Managers are here to guide you, helping you understand your options and navigate the process so you can access the support you need.
Our Case Managers work with individuals and families to:
- Identify and secure community resources
- Empower you to meet your basic needs and improve your overall wellness
- Connect you with services like housing assistance, legal aid, and food support
What can my Case Manager do for me?
While Case Managers do not provide counseling or mental health treatment, they can:
- Work with you to create a personalized self-management plan
- Coordinate care among your medical and behavioral health providersPrepare you for appointments to enhance your treatment outcomes
- Facilitate communication between your care teams
- Identify and direct you toward helpful community resources
- Help resolve barriers by arranging for additional or specialized services
- Manage treatment access and follow up on care for those with coexisting medical and behavioral conditions
- Implement preventative programs for behavioral healthcare needs
- Offer expertise for individuals with severe and persistent mental illness

Peer Support
At Kaniksu Community Health, Peer Support empowers individuals to make the best decisions for themselves and move toward their goals within their communities. Peers are a vital part of a recovery-focused system, offering support at every stage of the recovery journey.
What is Peer Support?
Peer Support gives you the opportunity to work with someone who has personal, lived experience with mental health, substance use, or trauma-related challenges. A Peer Support Specialist truly understands what you’re going through and can provide empathy, encouragement, and support as you navigate your own recovery path. Our goal is simple: To help you identify and build on your strengths and skills so you can overcome barriers and move forward.
How Peer Support Helps:
- Promotes recovery, resilience, and empowerment
- Uses a strength-based approach that emphasizes emotional, psychological, and physical safety
- Creates opportunities to rebuild confidence, control, and hope
What to Expect:
A Peer Support Specialist can work with you 1–3 hours per week to help you get “unstuck,” find motivation, and take meaningful steps toward your goals. We meet you where you are — whether that’s in our office, at a park, over a cup of coffee, or even on a walk. Wherever you feel most comfortable, we’ll be there to support you.

Stroll & Roll
A Peer Support Walking Group
Stroll & Roll is a welcoming peer support group designed to help you get moving, connect with others, and support your mental health in a relaxed, low-pressure environment. This group focuses on gentle movement, fresh air, and meaningful connection. Whether you prefer to walk or use a mobility aid, all ability levels and paces are welcome.
What to Expect
A relaxed, supportive group environment. A 30-minute walk or roll along a designated path in Bonner County. Opportunities to connect with others on a similar journey. A pace that works for you with no pressure or expectations.
Who Can Join
Stroll & Roll is open to individuals participating in Peer Support services. To join, you will need to complete an assessment and create a recovery plan with a Peer Support Specialist.
When & Where
Group walks take place at designated locations in Bonner County. Meeting times and locations may vary based on weather and group needs.
